Has a green label in the centre with, Recorded by W. Bate, David Street, Blenheim - Quite Parision, Ray Houlston (Nisch). The other side has, Recorded by W. Bate, David Street, Blenheim - Oh Say No More/Love In My Heart, Jean Williams, Godfrey Hislop. No. 153".

Written lightly in pencil on the cover is, "T. Woodward, in the donor's hand, plus the number "153", in a different hand.
